Please download and complete the intake form and bring it to your first appointment. You will need to complete an Authorization and Consent To Treatment Form when you arrive for your visit. You are not considered a client of Sapience Life Solutions until you and your provider have signed the Authorization and Consent to Treatment Form.
Cancellation Policy
You may cancel or reschedule an appointment at anytime as long as you provide 24 hours notice. If you cancel an appointment with less than 24 hours notice or do not show up to your appointment, you will be billed half the amount of the service on the first occasion. Future missed/cancelled appointments without 24 hours notice will result in you being billed for the entire appointment. Insurance does not cover this cost.
This policy is not meant to penalize you in any way. I exclusively reserve a full hour to accommodate your appointment and document your treatment. Therefore, if you cancel with less than 24 hours notice, I will be unable to fill that time slot and lose an hour from my work schedule. Please notify us by phone/voicemail or email and we will gladly reschedule your appointment.
Confidentiality & Privacy Policy
The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission.
Exceptions include:
- Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately. This also applies to pregnant women whose behaviors place an unborn fetus at risk.
- If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
- If a client intends to harm himself or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety.
